• A statement by you that: (a) you believe in good faith that uses which forms basis for the complaint is not authorized by law or copyright owner/agent thereof; and (b) under penalty perjury accurate complete knowledge at time notice given material complained against either belongs someone else’s rights reserved under law intellectual property rights own.
